Overview

Fairview Home Care and Hospice is looking for a Nursing Assistant or Home Health Aide that travels to the client’s residence. We service clients throughout the entire metro area.

This position will provide assistance with bathing/showering and personal cares.

This is can be part-time or full-time: .6, .8, .9, and 1.0 and is on the evening shift.

Responsibilities/Job Description

The Home Health Aide cares for patients following a written care plan and under the supervision of a Case Manager/Registered Nurse. They also provide direct patient care and homemaking services for home care and hospice patients. The Home Health Aide may also carry out selected exercise programs under the direct supervision of a Registered Physical Therapist. The Home Health Aide functions as a member of an interdisciplinary team. Fairview Home Care and Hospice services clients throughout the entire metro area.

Qualifications

Minimum License/Certification/Registration

Proof of Nursing Assistant or Home Health Aide Certification (not required to be current).

Minimum Experience

Minimum of 6 months (1,000 hours) experience as a certified Nursing Assistant or certified HHA within the last 2 years; or provide verification of one year current clinicals as part of an approved registered nursing program; or provide verification of 1 year current clinicals as part of a related health science program (PT, OT, SLP).

Other Requirements

Must have reliable transportation, valid Minnesota drivers license (or license from the state in which you claim permanent residence) and current and adequate automobile insurance policy.
