Responsibilities/Job Description

The Hospice Home Health Visit Nurse is a Registered Nurse (RN) responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The Hospice Home Health Visit Nures (RN) is responsible for performing these responsibilities in accordance with all policy, procedure and professional practice guidelines. TheHospice Home Health Visit Nurse (RN) coordinates care with members of the health care team and delegates responsibilities to others as appropriate.

Qualifications

Required Experience

  • One year recent RN experience

  • Computer skills: able to document effectively in Electronic Medical Record (EMR)

License/Certification/Registration

  • Current Minnesota RN License

  • BLS for Health Care Providers certification

Education

  • Bachelor’s degree in nursing

Preferred Experience

  • Previous RN experience in high-tech home care or hospice, med/sug, oncology, or critical care

  • License/Certification/Registration

  • Public Health Nurse (PHN) certification

Additional Requirements:

  • Reliable transportation

  • Valid Minnesota driver’s license

  • Current auto insurance

  • Excellent written and verbal communication skills

  • Excellent critical thinking and problem-solving ability

  • Good interpersonal skills

  • Knowledge of and ability to communicate with diverse patient populations

  • Time Management skills

  • Excellent teamwork ability

  • Knowledge of systems thinking
