Overview

The mission of the Care Transitions team is to screen all patients identifying those at risk of adverse health consequences, then providing those at risk with tools and resources that optimize and achieve health goals. This work integrates community resources with all next level of care referrals using shared decision-making conversations with the patient and family to identify the next steps of their journey along the continuum of care.

Responsibilities/Job Description

This position will identify and create action plans to meet the social, psychosocial, environment and/or discharge planning needs of patients and families who have complex health situations in order to meet their quality of life goals while adhering to the triple aim and maintining autonomy of choice.

Qualifications

REQUIRED:

  • Current MN Social Work license

  • Social Work degree - BSW

  • Three years experience in a health care setting

  • Excellent written & verbal communication skills

  • Efficient time management skills

  • Basic PC skills

  • Ability to communication with a diverse patient population

  • Excellent critical thinking and problem solving skills

Preferred:

  • 1 year medical social work

  • Social Work degree - MSW
