A newly-created position, Wellness Coordinator, is now available at York Gardens Senior Living located in Edina. The Wellness Coordinator will work directly with residents, their families, and the Nursing and Life Long Learning staff to ensure that psychosocial and physical needs of residents are met. This position will provide and document individualized cognitive assessments, and support Ebenezer’s dementia program. We are looking for a compassionate, highly skilled individual who wants to help our residents attain their highest level of independence and promote quality of life above all else. This position will be considered a Part-time, primarily day position, Monday-Friday.
Contributes to the development of the care plan, participates in care conferences and education sessions with families.
Participates in supervisory visits
Assists with hands-on training and coaching of care staff
Supports Director of Health Services as necessary
Applicant requirements include Bachelor’s degree in Occupational Therapy (OT) and current OT license in good standing
