Fairview Health Services Behavioral Health Clinician in Maple Grove, Minnesota

Overview

In 2015, the Agency for Healthcare Research and Quality named us as one of eight “exemplar” integrated behavioral health care models across the country. Integrated care is a result of an engaged care team and Behavioral Health Clinician providing patient-centered care addressing mental health and substance abuse conditions, health behaviors, stressors, social determinants of health, and ineffective patterns of health care utilization. In our clinics, integrated care is a “team sport.”
