Fairview Health Services Nurse Liaison/Field Nurse-Home Infusion; North Region in Minneapolis, Minnesota
We provide extensive home and outpatient infusion services to patients requiring IV therapy such as feeding by IV, infection treatment, chemotherapy, pain management and blood products/transfusions. Fairview Home Infusion is one of the largest home infusion organizations in the upper Midwest. It uses a care team structure—pharmacists, doctors, nurses, nutritionists, insurance experts—and customized operational services and reports to meet the needs of referring providers and patients. Our outpatient infusion centers provide chemotherapy and other infusion services along with clinical consultation and patient education. Specialized pharmacists work in collaboration with patients’ physicians and nurses to coordinate care at ambulatory suites throughout the Twin Cities and the surrounding Duluth area.
In addition to the Nurse Liaison role, this position will have additional responsibilites in the field seeing patients in the home, primarily in the mornings. Afternoons, this individual will act in the Nurse Liaison position. This position will be supporting North Memorial/Maple Grove hospitals.
The FHI Clinical Nurse Liaison develops referral source relationships to ensure smooth patient-centered transition to Fairview Home Infusion The Liaison provides education and consultation to discharge planners-and to patients and families as needed. Attends care conferences and rounds to provide Home infusion expertise and to manage the referral process on site. Accepts appropriate referrals, eliciting pertinent clinical and social data related to referral in order to determine need for service(s), and enters complete, accurate referral information directly into FHI EMR/. Interacts with all customers to deliver exceptional customer service. Assists facility and FHI staff in problem solving complex cases, interacts with managers and case managers as appropriate. Communicates benefit information to patients and hospital staff.
Facilitate and manage home infusion referrals.
Inform referral sources about current services, clinical outcomes, benefits, reimbursement policies and trends.
Seek appropriate home infusion referrals and assure fast, easy access to FHI..
Accurately and completely document referrals into software on laptop.
Exhibit excellent customer relations with clients, coworkers, referral sources, and others.
Ability to problem solve, plan, and resolve issues independently or in collaboration with coworkers or supervisor.
Call identified referral sources, physician offices, and patients
Promote FHI clinical services and programs to key referral sources in Fairview Hospitals.
Serve as a resource to healthcare facility staff regarding home infusion services
Establish and maintain an effective and smooth communication system between FHI and referral sources.
Follow up promptly on any concerns or problems.
Maintain an accurate and complete record or database of client contacts
Develop and initiate marketing presentations to clinics, hospital departments, and nursing staff
Meet with discharge planners and attend rounds to educate, assess patient needs and coordinate safe transition to home.
Consult with patients and families to explain programs, provide education and assess appropriateness and transition of the patient to the agency.
Attend care conferences upon request to discuss home infusion and assess appropriateness.
Coordinate with FHI RN Clinical Coordinators to provide a smooth transition of care.
Current Minnesota RN License
Minimum three to five years of home infusion experience
Knowledge of home infusion policies and procedure
Previous customer service or sales/marketing experience
