Fairview Health Services RN Case Manager - Hospice in Minneapolis, Minnesota

Responsibilities/Job Description

The Hospice RN Case Manager plans, organizes, coordinates and manages a caseload of hospice patients according to a plan of care. Performs advanced nursing interventions and education related to the patients' needs. Works collaboratively with interdisciplinary team members.

Territory is primarily Minneapolis, may be required to travel to other Metro areas intermittently.

Qualifications

Required

Experience

  • One year recent RN experience

  • Computer skills (able to document effectively in EMR)

License/Certification/Registration

  • Licensed as a RN and eligible to practice in Minnesota

  • Current CPR (BLS for Health Care Providers) certification

Preferred

Education

  • Bachelor’s degree in nursing

Experience

  • Previous RN experience in high-tech home care or hospice, med/sug, oncology, or critical care

License/Certification/Registration

  • Public Health Nurse (PHN) certification

Additional Requirements

  • Must have reliable transportation

  • Current Minnesota driver’s license (or license that matches your permanent residence)

  • Must carry current and adequate automobile insurance
