Responsibilities/Job Description

The Home Care Wound Ostomy Continence Nurse plans and delivers wound, ostomy, or continence services to patients using the nursing process of assessment, planning, intervention, evaluation and teaching. Provides consultation, education, and direct care for patients with needs in the areas of wounds, ostomies, and incontinence problems. WOC Nursing collaborates with physicians, nurses, and other health care disciplines to determine best treatment protocols, and facilitate quality outcomes for ostomy and wound care patients. WOC Nursing incorporates current research and best practice guidelines for the care of ostomy and wound patients.

The territory traveled for this position is the north end of Onamia to the south end of Brooklyn Park.

Work schedule: Monday through Friday, 8:00AM to 5:00PM. This position is benefit eligible.

Qualifications

Required:

Education

  • Bachelor’s degree in Nursing and graduation from an accredited WOC Nursing program.

Experience

  • One year recent RN experience in acute care, home care, hospice or long term care.

  • Completion of WOC Nursing Internship as part of WOC Nursing Program.

  • Ability to function in job role independently.

License/Certification/Registration

  • Licensed as a RN and eligible to practice in Minnesota.

  • Certified Wound, Ostomy, Continence Nurse (CWOCN) or Certified Wound Ostomy Nurse (CWON), or the ability to become certified within 1 year of hire.

  • BLS for Health Care Providers certification

Additional Requirements

  • Valid Minnesota driver’s license (or license that matches your current permanent residence).

  • Current and adequate automobile insurance policy.

  • Reliable transportation.

  • Computer skills (able to document effectively in EMR).

  • Interest and ability to lecture and provide staff education related to WOC nursing.
