Fairview Health Services Community Health Worker in Saint Paul, Minnesota
We are hiring Community Health Workers (CHW) with strong team and patient engagement skills that enjoy innovation, learning, and being a member of high performing teams. This position is located at our Andover Clinic in Andover, MN.
As a key member of the Behavioral Health Home (BHH) primary care team, the Community Health Worker supports health and wellness for patients with chronic mental and physical health conditions. In collaboration with the Primary Care BHH team, the CHW supports the health and functioning of the defined patient population. The community health worker works closely with patients/families, the care team, and community services to address patient healthcare and resource needs. Face to face services will be provided to BHH eligible / enrolled patients in their community and home.
3 years’ experience working on community health projects, programs, or initiatives
Community based leadership experience
Experience teaching or presenting information to groups
Experience, competence and sensitivity working with diverse communities
Community Health Worker Certificate
Associate’s Degree, Bachelor’s Degree or completion of CHW courses at accredited school
