Fairview Health Services Chemical Dependency Counselor in St. Paul, Minnesota

Overview

Provides primary case manager for assigned patients. Performs a variety of therapeutic and service assignments concerned with the treatment of alcoholism and other drug dependency in the primary, family and aftercare phases of the program.

Responsibilities/Job Description

  • Communicates all referrals, external sources to facilitate patient care and progress in treatment

  • Meets with social workers, county staff, probation officers, employee assistance counselors or other concerned parties as needed, with appropriate consents

  • Provides day-to-day provision of primary counseling services in accordance with program policies and procedures

  • Contributes to the development, implementation and evaluation of the patient’s master treatment plan in collaboration with other team members

  • Documents patient information in EHR and/or other electronic system in accordance with program standards, department policies/procedures

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards

  • Actively participates in creating and implementing improvements

  • Performs other responsibilities as needed/assigned

Qualifications

Required:

  • Bachelor's degree must meet one of the Minnesota Department of Human Services qualifications categories for counselor

  • Licensed Alcohol and Drug Counselor (LADC) through MN Board of Behavioral Health & Therapy

  • First Aid Certification through American Heart Association

  • CPR through American Red Cross or American Heart Association

  • 1 to 2 years experience in chemical dependency treatment field

  • Knowledge of application of self-help models (such as Alcoholics Anonymous)

  • Knowledge of the continuum of care for chemical dependency

  • May need to have current valid driver's license and access to reliable transportation
