As a Care Manager you will be part of a team that tracks the patient from point of entry into the hospital all the way to making sure they have everything they need upon discharge. Your work will ensure the achievement of desirable outcomes, appropriate length of stay and efficient utilization of resources. You will also influence increased patient and family involvement in their care plan, and provide education to patients, their families and hospital employees.
Maintains knowledge of, and complies with, all relevant laws, regulations and policies, procedures and standards.
Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
Develops, coordinates and implements clinical care management through partnership with Social Work Care Manager and Care Management Assistant in collaboration with clinical leadership, physicians, nursing staff, and other interdisciplinary clinicians.
Participates in psychosocial management of patients.
Facilitates plans for the transitions of patient care to the next level and location of care.
Partners with system care navigation resources and care navigators from other settings to provide continuity of care and effective transitions.
Maintains accountability for utilization management and communication with payers to assure continued stay authorization and assists with financial activities affecting the hospital stay.
Conducts review activities on a daily basis following InterQual guidelines.
Performs concurrent reviews to respond to payers.
Associate's degree from an accredited school of Nursing required
Bachelor's degree from an accredited school of Nursion preferred
Valid RN license with the MN Board of Nursing
1 to 3 years experience required
Assessment, Planning, Evaluation: Collects and analyses pertinent data and/or clinical findings. Incorporates clinical findings and scientific evidence in the development, coordination and implementation of an individualized plan of care to achieve desired outcomes. Evaluates progress towards goals and outcomes identifying opportunities for improvement and change.
Computer Skills Basic: Ability to use a computer and applications that are associated with performing basic work tasks (use a mouse, navigate in Windows, Outlook, etc).
Evidence Based Practice: Evaluates and/or assists in the evaluation of practice in relation to existing evidence identifying opportunities for the generation and use of research
Interpersonal Communication: Skilled in developing effective rapport with customers, co-workers, or families, actively listening to develop a positive connection.
Nursing Process: Utilizes the nursing process to provide holistic, compassionate, safe, high quality, population based care.
Patient Advocacy: Gathers information that reflects the patient's perspective; advocates and brings resolution to customer concerns; effective in mediating with third party payers on behalf of the patient.
Patient Safety: Demonstrates practices that support safety with recognition of variations in populations served, preventing harm to patients, families, employees, and themselves.
Professional Practice: Demonstrates behavior consistent with the standards, scope of practice, ethics, and characteristics of a licensed professional.
Systems Thinking: Understanding how things influence one another other within a whole; carefully balances the good of the department or site with the entire organization.
