Take your nursing skills directly to your patient's home! With the changes in healthcare where patients are being discharged more quickly from the hospital there is a much greater need for home care. This growing field is both challenging and rewarding! As a Visit RN for HealthEast Home Care you will enjoy working directly with your patient and their family during a time where your knowledge and skill can make a world of difference. The American Nurses Association (ANA) appropriately described home health nursing as a "synthesis of community health nursing and selected technical skills from other nursing specialities". You will use all of your nursing knowledge and skill in this very rewarding specialty!

Responsibilities

  • Maintains knowledge of, and complies with, all relevant laws, regulations and policies, procedures and standards.

  • Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.

  • Delivers effective and appropriate home nursing care, establishing key relationships with patients and family members for the purpose of advocacy and coordination of care.

  • Collaborates with RN Case Manager and other health professionals as appropriate in determining patient care goals.

  • Educates the patient/caregiver how to self-manage their disease on a day-to-day basis in collaboration with the RN Case Manager.

  • Recognizes and reports signs of impending physiologic and/or psychological patient changes to the appropriate physician and RN Case Manager to implement appropriate patient care interventions.

  • Recognizes, evaluates and documents needs of patient and family to provide the most effective and appropriate patient home health care.

Qualifications

Experience: Experience in acute/skilled facility or home care with the past 2 years required. 2 years experience preferred.

  • Assessment, Planning, Evaluation: Collects and analyses pertinent data and/or clinical findings. Incorporates clinical findings and scientific evidence in the development, coordination and implementation of an individualized plan of care to achieve desired outcomes. Evaluates progress towards goals and outcomes identifying opportunities for improvement and change.

  • Computer Skills Basic: Ability to use a computer and applications that are associated with performing basic work tasks (use a mouse, navigate in Windows, Outlook, etc).

  • Evidence Based Practice: Evaluates and/or assists in the evaluation of practice in relation to existing evidence identifying opportunities for the generation and use of research

  • Nursing Process: Utilizes the nursing process to provide holistic, compassionate, safe, high quality, population based care.

  • Patient Safety: Demonstrates practices that support safety with recognition of variations in populations served, preventing harm to patients, families, employees, and themselves.

  • Professional Practice: Demonstrates behavior consistent with the standards, scope of practice, ethics, and characteristics of a licensed professional.
