Fairview Health Services Staff Development Coordinator-Home Care in St. Paul, Minnesota


The Staff Development Coordinator acts as a resource for homecare clinicians. Instruct and mentor new nurses in their roles at HealthEast Home Care. Provide ongoing education and mentoring to clinicians throughout their career trajectory with Home Care. The Staff Development Coordinator collaborates with management and education to ensure clinicians learn and implement best practice measures and patients receive high quality care.


  • Collaborates with Education Team and Clinical Manager to develop a learning plan based on identified learning needs and style of the new clinician

  • Implements the identified individualized orientation plan with the clinician in a collaborative and respectful manner

  • Demonstrates use of strategies for successful precepting of the culturally diverse clinician

  • The expectations of the job responsibilities for their position

  • HEHC role-related resources, policies, procedures

  • Uses standardized agency checklist to track and provide competencies and progress in orientation plan

  • Documents orientation and competency for the clinician’s personnel file through the use of provided tools and checklists

  • Evaluates the skills of the clinician through direct observation and feedback from Clinical Manager and peers

  • Provides the clinician with specific feedback related to their progress and general performance

  • Identifies and documents communication to Clinical Manager via email regarding any barriers to expected progress, and collaborates with Clinical Manager to resolve barriers

  • Makes formative and summative evaluations for clinicians

  • Demonstrates ability to diagnosis learning and performance issues and implement/documentation response to corrective action plans

  • Positively and effectively models and reinforces agency identified behaviors that enhance an exceptional customer experience

  • Assesses effectiveness of new clinician application and verifies new clinician is able to assess and document outcome of best practice implementation

  • Demonstrates solid critical thinking skills related to the process of care and facilitates development of same characteristic in the new clinician

  • Attends orientation sessions as assigned to stay current with agency process, skills, and employee expectations

  • Leads specified orientation classes and continuing education/training sessions for new and established employees

  • Evaluates learner progress and identifies trends that indicate potential gaps in training process

  • Identifies strategies for improvement and participates in development and training as requested

  • Facilitates socialization in the organization and the workgroup

  • Confirms new nurse demonstrates behaviors indicative of understanding and ability to manage expected inter- departmental collaboration

  • Assures new clinician has exposure to selected assignments to enable job competence by 6 months, and proficiency by 12 months

  • Works collaboratively with PCSC Team and field clinicians to select, schedule, and verify that required patient care experiences are obtained

  • Assures evidence of required orientation competencies is documented in routed to new clinician file

  • Collaborates with Clinical Manager

  • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards

  • Actively participates in creating and implementing improvements

  • Performs other responsibilities as needed/assigned


  • Bachelor's degree in Nursing required

  • Valid Basic Life Support (BLS)

  • Registered Nurse MN (RN) through MN Board of Nursing

  • Minimum of 1 year RN experience with HealthEast Home Care

  • 2 years recent RN experience in acute care, home care, hospice or long term care.Ability to function in job role independently

  • Successfully completes core competencies of their position and continues to meet department and role responsibilities

  • Proficient in all clinical skills needed for home care or hospice

  • Clinical documentation meets regulatory requirements and is thorough and timely on a consistent basis

  • Reliable transportation

  • Valid Minnesota driver’s license (or license that matches your current permanent residence)

  • Current and adequate automobile insurance policy

  • Document effectively and train all disciplines in the use of EMR

  • Achieves thoroughness and accuracy when accomplishing a task
