Following a written care plan and under the supervision of a Case Manager/Registered Nurse, the Home Health Aide provides direct patient care and homemaking services for home care and hospice patients. Under the direct supervision of a Registered Physical Therapist they may carry out selected exercise programs. The Home Health Aide functions as a member of an interdisciplinary team.
This position is 2-3 days during the week and an every other weekend rotation. The expectation is that a person will be available between the hours of 3:00pm - 11:30pm and work an 8 hour shift within that particular time-frame.
This position is travel intensive. Fairview Home Care and Hospice services clients throughout the entire metro area. Staff can expect to be traveling several hours per day based on assignments given.
The majority of travel will be throughout the Central & North Metro area
This position is benefit eligible.
Proof of Nursing Assistant or Home Health Aide Certification (not required to be current).
Minimum of 6 months (1,000 hours) experience as a certified Nursing Assistant or certified HHA within the last 2 years; or provide verification of one year current clinicals as part of an approved registered nursing program; or provide verification of 1 year current clinicals as part of a related health science program (PT, OT, SLP).
Must have reliable transportation, valid Minnesota drivers license (or license from the state in which you claim permanent residence) and current and adequate automobile insurance policy.
