Job Description:

Within the scope of practice, the Social Worker assists hospice patients and their families in maximizing the utilization of health care and community resources in order to support the patient at home or in an alternative living situation and to achieve their optimal level of wellness. The Social Worker is responsible to identify and create action plans to meet the social, psychosocial, environmental and/or discharge planning needs of patients and families.

Provides a range of generalist social work interventions to assist patients and families reach their goals. Advocates for individual patients, familied and patient groups from social work theoryand practice perspectives.

Job Qualifications:


Current Minnesota Social Work licensure at the LGSW, LISW or LICSW level

One year recent post-educational social work experience in health care setting

Valid Minnesota drivers license (or license that matches your current permanent residence)

Current and adequate automobile insurance policy

Reliable transportation

Computer skills: able to document effectively in EMR


Master's degree in Social Work


Two or more years of social work experience in hospice, homecare, hospital, or related health care setting.

Additional information

Schedule is casual. Required to work a minimum of 1 weekend per month.
