Job Description:

The Patient Services Coordinator provide timely staffing for patient cases keeping the patient first; keeps complete, timely and accurate staff records; communicates assigned schedules to staff and clientele.

Coordinate assignment of referrals with RN Clinical Coordinator or Clinical Manager. Staff each case with the appropriate employee(s) consistent with staffing guidelines and striving for customer satisfaction.

Take patients complaints in a courteous and caring manner, forwarding to Clinical Manager for resolution.

Reschedule cases as needed. Document all staff absences per agency protocol and communicate unusual trends and patterns to employee's supervisor.

Utilizes Horizons scheduling product to create service orders, generate calendars and maintain accurate schedules for client and employees.

Communicates missed visits to physician and provider offices per agency protocol.

Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.

This position is located in Burnsville. The schedule is Mon-Fri, 8:30am -5pm, with occasional weekends possible.

Job Qualifications:

Minimum Experience:

Minimum six months of previous experience in service industry/profession.

Experience with Window's based programs.

Experience with general office equipment.

Preferred Experience:

One year of staffing experience preferred; home care/hospice experience helpful;

medical background desirable.

Preferred Education:

Vocational/Technical Training or Associate Degree in the medical field
