In collaboration with patients and families, the Physical Therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes. The physical therapist for this position will primarily provide outpatient therapy services for adults.
Partnering with the patient and family while performing the evaluation/re-evaluation
Providing patient education based on as assessment of learning needs of patient/care giver
Maintaining an individualized plan of care, related to the patient’s needs,
Continually reassess resource utilization as patient progresses towards goals
Providing safe and effective intervention, within scope of practice, according to patient needs
Fairview Lakes serves outpatients on the Wyoming campus as well as in northern service area clinics in Chisago, North Branch and Pine City. This position will include core outpatient hours at a northern clinic with location to be determined based on clinical experience of candidates and alignment with programmatic needs. Experience with orthopedic populations is preferred. In addition to orthopedics, experience with neurologic conditions and/or vestibular rehab is a plus. Select locations offer pediatric therapy thus this would be a practice opportunity. Fairview Lakes includes a team of over 60 engaged physical therapists, occupational therapists, speech-language therapists, exercise physiologists and support staff. BS in Physical Therapy
PT registration/licensure in State of Minnesota
Current Basic Life Support (BLS)
5 years of post-graduate PT experience in a outpatient setting
Masters degree in Physical Therapy
