Job Summary:

Contribute to Population Health strategy within the Value Based Service Organization, implement system wide tactics to improve the quadruple aim metrics. Support the management of the networks for multiple plans and support productive relationships with physicians, providers, and payers. Manage and effectively support the operation of appropriate contract relationships to ensure a viable revenue stream for the company, as requested by the System Director. Lead tactical execution supporting the system strategy to ensure risk based performance metrics are achieved through the entire Fairview System. Support System Operations Center as requested. The Manager of Population Health Strategy and VBSO Operations, will report directly to the System Director Population Health Strategy and VBSO Operations.

Job Expectations:

Manage programs and relationships, key stakeholders and partners to maximize contract performance and delivery of services to Fairview System.

Support management of provider networks to serve VBSO, network members and communities served.

Support the Population Health Governance Committees by implementing and executing programs related to the tactical plan, within the VBSO and clinical infrastructure to achieve goals.

Partner with data and analytics functions to acquire data for the risk based scorecard for all population health metrics as necessary.

Deploy process improvement activities to accelerate and drive an agenda of operational efficiency and clinical excellence. Suggest and drive change based on conclusions derived from analytic data.

Implement risk stratification and acuity coding efforts for necessary populations.

Support the Director of Care Coordination with program and project management of quality interventions and appropriate tracking of goals as available.

Assist VBSO leadership in maintenance of an empowering environment, and being a continually evolving organization capable of being more nimble in a rapidly changing healthcare environment. Demonstrate related initiatives by example.

Deliver program administration services for VBSO initiatives and scope of responsibilities as requested by the System Director.

Ensure all relevant information is communicated in a timely and professional manner, vertically and laterally within the organization.

Job Qualifications:

Required:

Education:

BS in business or health related field (e.g. health administration, biomedical engineering, health care economics, nursing, health data analytics)

Experience:

3+ years progressive experience in a healthcare setting, including management of complex multidisciplinary programs supporting strategy and operations

Preferred:

Education

Masters level degree in Business or Health Administration

Experience

5 - 10+ years in leadership roles, including management of complex multidisciplinary programs supporting strategy and operations

Progressive leadership in healthcare industry (e.g. provider group, care system, suppliers, payers, data analytics) with record of significant accomplishment

Strong preference for experience working in public health, population health operations / strategy, payor products / contracting, Government payer programs regulations or large multidisciplinary care system
