The RN Case Manager is responsible for coordinating the delivery of all care/services appropriate to the needs of the patients of all ages and their families. Complete a thorough physical and psychosocial assessment of the patient and identify effective interventions with the patient, family under the direction of physician to address the specific care goals. Maintain complete and timely records, verify authorizations. Provide clinical supervision as appropriate.
The RN Case Manager will see patients primarily in the East Metro.
East Metro territory includes: E. Minneapolis, St. Paul, Inver Grove Heights, Eagan, Rosemount, Apple Valley, Farmington, Lakeville, Roseville, New Brighton, Columbia Heights, Maplewood, Oakdale, Woodbury, White Bear Lake, and Stillwater.
Work schedule is Monday through Friday 8:00AM to 5:00PM, plus every 6th weekend and 1-2 holidays a year.
Six months recent RN experience in an acute care, home care/hospice or long term care setting
Computer skills: able to effectively document in EMR
Licensed as a RN and eligible to practice in Minnesota
Current CPR (BLS for Health Care Providers) certification
Valid MN driver's license (or license that matches your current permanent residence)
Current and adequate auto insurance
Previous RN experience in high tech home care or hospice, med/surg, oncology or critical care
BSN or current active pursuit of BSN degree
Public Health Nurse (PHN) certification
