The Mental Health RN Case Manager is responsible for coordinating the delivery of all care/services appropriate to the needs of the patients of all ages and their families. Complete a thorough physical and psychosocial assessment of the patient and identify effective interventions with the patient, family under the direction of physician to address the specific care goals. Maintain complete and timely records, verify authorizations. Provide clinical supervision as appropriate.
The Mental Health RN Case Manager provides acute skilled mental health nursing care to patients in their homes. Serves as consult/educator in Mental Health RN role.
Will see home care medical and mental health patients, primarily in the South metro. Work hours are Monday - Friday, 8:00AM - 5:00PM. No weekends or holidays!
MN Nursing License and current CPR (BLS for Health Care Providers).
RN with Master's degree in psychiatric or mental health nursing: Recent (within last 3 years) nursing experience in acute treatment unit in psychiatric hospital, psychiatric home care, partial psychiatric hospitalization program, or other psychiatric services required.
RN with Bachelor's degree in nursing: One year of recent (within last 3 years) nursing experience in acute treatment unit in psychiatric hospital, psychiatric home care, partial psychiatric hospitalization program, or other psychiatric services required.
RN with Associate Degree in nursing or diploma: Two years of recent (within last 3 years) nursing experience in acute treatment unit in psychiatric hospital, psychiatric home care, partial psychiatric hospitalization program, or other psychiatric services required.
Preferred Qualifications and Experience:
BSN or MSN degree
Two plus years RN experience in a med/surg., critical care, oncology or high tech home care/hospice setting.
Basic computer skills, reliable transportation, valid MN driver's license, current and adequate auto insurance.
