The Registered Nurse Patient Care Management Coordinator is responsible for the assessment, planning, delivery, and evaluation of nursing care for all assigned patient population(s). The RN is responsible for performing these responsibilities in accordance with all policy, procedures, professional practice guidelines and the MN Nurse Practice Act. The RN coordinates care with members of the health care team and delegates responsibilities to others as appropriate.
This position provides comprehensive care transition management of patients along the health care continuum, as appropriate from referral and case finding. This position assesses patient's needs, evaluates progress, facilitates decision-making and collaborates with the health care team during the patient's episode of care as well as responsible for creating a plan of care that mitigates potential adverse consequences and promotes engagement towards success of their health care goals. Responsibilities include the identification of resources, facilitation of options for movement along the care continuum, and identification of transition care needs. This position identifies and resolves barriers which may hinder effective transitions. This position is measured on the triple aim for quality, cost efficiency and patient experience.
The applicant needs to be able to work effectively with the healthcare team across the care continuum, have extensive assessment skills both pathophysiological and psychosocial. Also, needs to be a strong informal leader with conflict resolution skills. Strong problem solving skills and communication skills along with comfort with ambiguity. Strong knowledge base in understanding their responsibility throughput management and escalating delays.
This is a day shift position, 7:30-4:00pm and includes every other weekend rotation (8 hour shifts). 548 hours every two weeks (.6FTE), and this position is benefit eligible
Every other weekend rotation as listed below
week one: Friday 8 hours, Saturday and Sunday 12 hours
week two: Tuesday and Friday 8 hours shifts
MN RN license by start date
5 years clinical RN experience in a hospital setting
Critical thinking skills
Case management background
One year care coordinator/care management experience
Public Health Nurse license
