Job Description:

We are hiring are currently hiring Social Work Care Coordinators to fill a key role in Behavioral Health Home Certified Primary Care Clinics. We have three openings available at the Burnsville, Wyoming and Princeton clinics.

As a key member of the Behavioral Health Home (BHH) primary care team, the Social Work Care Coordinator (SWCC)

supports health and wellness for patients with chronic mental and physical health conditions

In collaboration with the Primary Care BHH team, the SWCC supports the care transition and coordination needs of the defined patient population

Works closely with patients/families, the care team, and community services to address patient healthcare and resource needs

A combination of face to face and telephonic services will be provided to BHH eligible / enrolled patients to meet improve patient experience, quality of care, and reduce the total cost of care

Job Qualifications:


Bachelor's Degree in Social Work

MN state license on Board of Social Work



Knowledge of behavioral health conditions

We may look at GPA as criteria

Experience with Electronic Health Record systems
