Job Description:

This position provides clinical social work services to pediatric patients and their families in our outpatient clinics (ie:Discovery Clinic, Explorer Clinic, Sickle Cell, Eye Clinic, Long Term Survivorship Clinic, etc) and occasionally the in-patient hospital units. This position will serve a variety of medical diagnoses and treatments including auto islet transplant. Primary responsibilities include, but are not limited to, providing emotional support, crisis intervention and counseling for pediatrics, adolescents, adults/parents/guardians to cope with the social and emotional problems relating to the patient's medical diagnoses. Complete psychosocial assessments and provide short term case management when needed. Guidance around and referrals to appropriate financial, community and insurance resources. Assists with mandated reporting, ongoing management, and coordination of community resources to victims of abuse, neglect, domestic violence, etc. Complete necessary documentation and maintain clinical record for each client interation.

Works in collaboration with all members of the treatment team, including physicians, nurses, and case managers to further their understanding of significant social and emotional factors contributing to the patients well-being.

Job Qualifications:

Master's degree in Social Work and experience in health care.

LGSW required-prefer LICSW
