Serves as the primary care coordinator for patients in the Post-transplant phase of care management providing life time care to patients on immunosuppression therapy. Serves as a liaison with transplant physicians.
Duties to include:
Collaborate with MD, interdisciplinary team members, community members, patients, families, and significant others to plan, manage, evaluate care systems and resources.
Serve as liaison with transplant physicians to implement medical management of condition changes, diagnostic test results, lab results, based on knowledge of patient and patient condition.
Acts as liaison with community MD and pharmacists/home care/IV to ensure continuum of care is provided.
BA/BS in Nursing
MN RN Licensure
3-5 years acute care experience in ICU, CCU, Transplant, Acute Care
Background in transplant nursing preferred
