Fairview Health Services RN Anticoagulation Nurse in Wyoming, Minnesota


The anticoagulation RN will provide monitoring of appropriate levels for anticoagulation patients at Fairview Lakes Medical Center in Wyoming, MN.

  • The department is open: M-F; 8:00 a.m. to 4:30 p.m.

  • .4 FTE on the day shift (working 2 days per week)

  • Flexibility needed to pick up extra day shifts when needed

Responsibilities/Job Description

Job Description

Provide ongoing monitoring of anticoagulation patients. Provide necessary education and support to patients and caregivers while performing ongoing case management. Monitor progress toward established goals to achieve optimal outcomes. Maintain complete, timely and accurate clinical documentation for each patient.

Core Responsibilities

  • Provide initial and ongoing assessments of patient, including evaluation of drug therapies, medical conditions, diet and compliance

  • Provide monitoring of appropriate levels for anticoagulation patients

  • Communicate effectively with the patient, hospital home care, nursing homes, clinic personnel, pharmacies and physicians regarding care

  • Provide education and learning opportunities to pharmacy students on rotation


Required Qualifications

  • Current MN RN License

  • One year RN experience

Additional Requirements

  • Excellent organizational and documentation skills, orientation to detail and ability to handle multiple tasks in a fast paced environment

  • Strong computerized medication document experience

  • Excellent communication and interpersonal skills

  • Demonstrated commitment to excellent customer services and service excellence

  • Must be able to work independently and also be a supportive team member

  • Respect the confidentiality of all information, including information regarding patients, staff members or Fairview business issues, and share information only with those who need to know.

Preferred Qualifications

  • BSN level RN or equivalent in a healthcare related field

  • Previous Clinic experience with anticoagulation medications

  • Telephone clinic triage experience helpful
